At the time of the visit, the following was observed: Student A was sleeping in the crib and was observed to be wearing a bracelet and earrings while they slept. Student B was sleeping in the crib and was observed to be wearing earrings while they slept. Jewelry of any kind must be removed prior to placing a child to sleep, unless the child's parent has given the program written consent to leave jewelry on during sleep. At the time of the visit, through a review of both files, neither student had written consent to wear jewelry while they slept.

At the time of the visit, Room 6 had a whiteboard listing students names and diagnosed conditions uncovered. An educator in the classroom identified they are in the process of memorizing all conditions as it is in the new school year; the educator covered the list at the time of the visit. A list of allergies and/or other emergency medical information provided by the parent for each child and a list of all emergency or life saving medications that specifies to which children they belong must be posted in a manner that protects the privacy of each child.

At the time of the visit, in a review of two students IHCPs and the corresponding Medication Consent documentation, the documentation was not fully complete. The programs self created medication consent document was missing "directions for storage". One student was also missing their prescribing physicians phone number. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.

At the time of the visit, the following was observed: Room 6 had multiple ceiling tiles with water stains and a lose extension cord by the computer in the classroom. Room 3 had multiple ceiling tiles with water stains. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning. All electrical cords must be arranged so they are not a hazard to children. The Infant/Toddler outdoor area was observed to have a small climbing structure that did not have a use zone of six feet; at the time of the visit it had a use zone of 3.5 feet. The first Preschool play structure was observed to have peeling and chipping paint on the handrails and had an area by the slide that where the corner was lifting up. The second Preschool Play structure was observed to have chipping and peeling paint on two ladders and was observed to have small black dots covering large portions of the play structure; it was unknown what the small black dots were. The outdoor space must be free from hazards including but not limited to chipping, peeling or flaking paint, and weather-related and environmental hazards.
