The medication consent for (REDACTED) was not signed by the pediatrician.
In one room, books and a bin were stored on the diaper changing table. In the same room the diaper changing paper was stored directly beside the hand washing sink.
There was a ground insert stuck through the top of the fence. The end had sharp tips.
In one infant room, two empty cardboard boxes were stacked under a full box of diapers. This was tippable and accessible to children.

There was flaking paint in several rooms. One room also had a small amount of exposed plaster.

In one room, the attendance was not completed to indicate the time of arrival of children. In another room, attendance indicated six children were present when seven were actually in attendance.
