The following was observed: 1) Despite the EEC essentials that were completed on 6/26/2025, Certified assistant {REDACTED] did not have evidence (copies of training certificates) in the file of the require hours per year since the license cycler started back in February 2023.

During the visit, the following was observed: 1) EEC regulation require attendance records indicating children's arrival and departures times. During the visit, the weekly attended sheet did not indicated arrival times for today 1/29/26 for the 4 children that were present. 2) During the visit, assistant [REDACTED] was already present and working at the program upon Licensor's arrival. During the course of the visit, Certified assistant [REDACTED] daughter arrived and joined the program. Educator did not have attendance records for both assistants indicating sign in times for today 1/29/2026.
