Garage door cannot open and has a lock installed to prevent access. Observed a flat screen television on a stand with wheels. This stand poses a tipping hazard. Discussed attaching the TV to a wall mount.

One child with a [REDACTED]. Allergy list is posted. Discussed the [REDACTED] being out of reach of the children, but in easy reach of the educator. Individual Health Care Plan (IHCP) is complete in child's file and signed in May 2022. Medication consent form is filled out, but not signed and dated by the parent. This form must be signed and updated every year by the parent.

The educator submitted a child record checklist. Reviewed five children's files. Educator does not use the annual update form. Parents must review their child's paperwork and sign off annually that the information in their child's packet has not changed. [REDACTED]documentation was up to date.
Emergency card pages located in the first aid kit were out of date for some children. These pages must be reviewed and updated annually by the parents.
Observed a common towel in the bathroom and a box of tissues, but no paper towels. A common towel must not be used to dry hands after handwashing. Liquid soap is accessible.
Trash can in the bathroom did not have a lid.
One child with [REDACTED]. Allergy is posted, but he location of the[REDACTED]is not. Discussed the [REDACTED} being out of reach of the children, but in easy reach of the educator. The child does not have an Individual Health Care Plan (IHCP) in their file. This form must be filled in and signed by the child's pediatrician. The parents must also sign the medication consent form in the event the child must receive the [REDACTED] at the program.
