The following was observed during the visit: -In INFANT 1, one crib had a white noise machine hanging from the top rung of the crib and a second crib had a bib hanging from the top rung of the crib. Both cribs had sleeping infants. -In INFANT 2, one crib had a bib hanging from the top rung of the crib. An infant was sleeping in this crib. -In INFANT 3, one crib containing a sleeping infant had a crib sheet that did not fit properly as evidenced by wrinkles throughout the whole sheet.

The following was observed to be accessible to children: -In TODDLER 4, an unlocked drawer next to the sink contained an open box of gloves, loose sandwich bags, adult hand lotion, and a loose phone charger with cord longer than six inches. -In the TODDLER 4 BATHROOM, two drawers contained loose gloves, loose trash bags, and a bottle of Lysol. -In the men's single bathroom that was unlocked, an aerosol Febreeze bottle was found. -In PRE-K the purple contact paper adhered to the back of the bookshelf in the play kitchen area was ripped and peeling off, the blue boombox cord was loose and longer than six inches, an unlocked drawer to the left of the sink contained two pairs of adult scissors, and a package of opened wipes was sitting on top of the toilet paper holder in the bathroom. -In PRESCHOOL 2, an unlocked drawer next to the sink contained two pairs of adult scissors, an unlocked a cabinet under the sink contained a bottle of Lysol spray and loose bags, and the unlocked storage closet contained a purse on the floor. -In the shared PRESCHOOL 2 and PRESCHOOL 1 BATHROOM, an open box of gloves, two packages of opened wipes, hand sanitizer, an aerosol spray of Febreeze, a plunger and toilet brush were found. -In PRESCHOOL 1, the walls where the sleep cots are stacked had a large amount of chipping paint, an unlocked drawer next to the sink contained a hot glue gun with a loose cord longer than six inches and a package of Wet Wipes. -In the shared TODDLER 1 and TODDLER 2 BATHROOM, an opened container of gloves, a bottle of hand sanitizer, and many opened bags of wipes were found on top and on the shelves of the bookshelf. -In TODDLER 2, an unlocked drawer to the right of the sink contained loose Ziploc bags. -In the unlocked STORAGE CLOSET near TODDLER 2, one shelf contained an open bag of pinto beans and an opened bag of white rice, and the doors to the mechanical room were open ajar. -In the TODDLER 3 BATHROOM, a plunger was next to the toilet and a drawer contained an opened box of gloves. -In the unlocked KITCHEN, soap dispenser refills in bags were under the sink in an unlocked cabinet as well as loose plastic bags.
