Allergy list is not posted as required. Two children with allergies (eggs and nuts, eggs) have EpiPens located at the program. The location of the EpiPens should be included on the list.
Two children with allergies and EpiPens. One child has an Emergency Care Plan signed by the pediatrician. However, this child needs a new medication consent form signed by the parent as the form on file expired in December 2025. The other child does not have an Individual Health Care Plan OR Emergency Care plan signed by the doctor and no signed medication consent form from the parent.
Reviewed 4 children's files. Two children need new annual update forms signed by the parents as they are both over a year old. One child is missing a current physical and list of immunizations.
