Two school-age children were observed being cared for in the program director's office. The program is not licensed to care for children of school age.
A review of the Pre-Kindergarten attendance showed seven children signed in while there were eight children in the classroom.
The preschool classroom did not have art, writing materials, or sensory materials accessible to children.
1. The baseboard in the Pre-Kindergarten bathroom is falling off. 2. The preschool bathroom had water-stained ceiling tiles
Chipped paint was observed in several areas of the Preschool and Pre-Kindergarten classrooms.
1. A plunger was observed stored in the Preschool bathroom within reach of children. 2. The sharp screws securing the dramatic play kitchen in the infant/toddler room had come out of the wall and were accessible to children.
An educator did not follow the two step process of washing and then disinfecting the diaper changing area after a diaper change. Another educator did not wash their hands or the child's hands after a diaper change.
The following was observed: 1. An educator did not complete the two-step process of washing and disinfecting a diaper changing table in the Toddler classroom 2. An educator did not wash their hands or a toddler's hands after completing a diaper change 3. There was no handwashing soap in the Pre-Kindergarten bathroom

A review of the staff record's checklist showed 6 staff members did have not completed their first aid certification.
Through a review of the children's record checklist, the following was noted: 1. 2 students did not have record of a physical and 1 was expired 2. 6 children had expired first aid/emergency consent forms 3. 5 had expired transportation plans 4. 10 students were missing evidence of lead tests
The toddler A classroom did not have an attendance.
Both toddler classrooms were lacking a variety of equipment made available for the children such as art, dramatic play, science and sensory.
The window sill near the library area in the Toddler B classroom has chipped paint.
1. An educator in the Toddler A classroom was observed not washing their hands after wiping a child's nose. 2. The Toddler B classroom did not have handwashing soap available.
A toilet brush was observed stored within reach of children in the Toddler B classroom.

The following was observed while conducting a health and safety inspection: * Broken plaster (dented) was observed in the Toddler A wall. *The vent in the Preschool classroom was dirty. * The baseboard was peeling off the wall in the preschool bathroom.
The exit leading from the preschool classroom to the playground did not have evacuation procedures posted.
*A closet door was observed open where Ice melt was stored within reach of children *Toilet brushes and a plunger were observed stored within reach of children in the preschool and Toddler B bathroom.
The mulch on the playground did not have the required nine-inch depth, not did two climbing structures have the required six feet fall zone.

A review of two IHCPs showed the following: * One student's IHCP was missing documentation of possible side effects off medication, consequences if the child does not receive the medication, who was trained in the IHCP and who trained the individual. * The second IHCP was missing who was trained on the IHCP and who trained the individual on it.
An infant educator was observed pushing cribs back and forth forcefully in order to “rock” the children to sleep. The infants were being moved roughly around the crib due to this action. The educator was told by the Licensor three times to stop.

Individual Health Care Plan (IHCP) - R Apr 2025
A review of the children's record checklist showed: * 4 students files were missing First Aid Emergency consent forms * 5 files were missing physicals *5 files were missing face sheets * 5 files were missing evidence of immunizations. A review of one student's file showed the program was not using the correct transportation authorization and plan form. The program director indicated the paperwork was collected but not marked on the checklist.
A review of the infant attendance showed two children were not signed into the classroom.
The staff schedule was not posted
A sharp screw like object was found sticking up from the base of a student's cubby.
A health and safety inspection of the playground showed the following: * The swing set/climbing structure did not have the required 12 feet on either side of the swings, nor did it have the adequate depth of bark mulch. * The geo climber did not have the required six feet all around.
A residential swing set was observed on the playground that is used by both toddlers and preschool age children. The swing set is intended for use by 3-6 year olds.
An educator was observed changing a child's diaper without washing the child or her hands after the diaper change.
A review of a student's IHCP showed a missing medication consent form for an [REDACTED]
Handwashing wasn't completed by student or educator following a diaper change.
