The program did not have the diapering mat accessible at this visit. The diapering mat must be kept in an area that is easily accessible. Toileting opportunities were provided as needed.
The door to the hatchway near the main entrance needs to be replaced. Chicken feces was located throughout the pathway to the main entrance. All areas must be clear of feces. The water temperature was exceeding 120 degrees F. The water temperature was tested and reading at 130 degrees F.

The program was exceeding adult-child ratios. The program was caring for seven children of whom none were of school age. The educator must ensure the program maintains within license capacity. Please indicate what plan has been put in place to ensure program will stay within license capacity.

Progress reports were not up to date. The program must ensure progress reports are completed in a timely manner.
The program was exceeding license capacity. The educator was caring for seven children of whom none were of school age.
The rabbit food was easily accessible. The food must be placed in an area that is at least four feet high or removed from the licensed space.
The educator added climbing playground structures to the outdoor play area that require fall zones. The program did not use proper spacing between the play structures. The spacing between structures must meet the CPSC guidelines.
The program has a dome climb structure outdoors. The dome must be removed and cannot be used with the daycare children. The sandbox must be protected with a covering when it is not in use.
This change of location visit inspection recorded no violations or advisories.
Five of the children's records were reviewed at this visit. 1.) Two of the records were missing the updated medical information. 2.) One record was missing proof of lead exam. 3.) One record was missing the date of admission. 4.) The general permission of the enrollment records must list walks around the driveway/neighborhood.
Cleaning materials were located at the bottom of the bathroom closet, the door to the closet did not include a lock. Cleaning materials must be placed out of the reach of the children or the door must include a lock.
