Persons on Premises
There are two persons on the premises on a regular basis that have not been approved by EEC as they have not completed the BRC process. Both individuals were on the premises on the day of the visit and the educator stated they come by 1-2 times a week.

The indoor space contained the hazards including but not limited to: wires in the kitchen near the fridge were accessible, there were suitcases and other hazards in the entryway preventing the room from being used by the children.
There was some confusion regarding which refrigerator was being used for childcare, the one in the basement or the one in the garage. The refrigerator in the basement did not have a thermometer and the one in the garage was not accessible due to the pathway being blocked.

The educator is not keeping accurate attendance records. The Educator’s infant child has been attending the child care since February and has not been listed on the attendance. Additionally the Educator’s school age child attends the child care after school and is not listed on the attendance.
Although the Educator was in compliance at the time of the visit, 4 children, she has been consistently over enrolled. The Educator’s licensed capacity is six, the Educator has been caring for as many as 8 children at one time.

The attendance is not accurate. The Educator stated she was closed on 11/7/2025 for a training. The attendance records show 5 children signed in/out on that day.
The first aid kit that is portable and goes outside with the children did not contain a CPR mouth guard. There is a CPR mouthguard with the first aid kit indoors
The temperature in the fridge was 48 degrees
There is chipping/peeling paint throughout the home: on the door ways and the bannister leading upstairs
The trash in the kitchen was on the floor and uncovered. There was rust on the heating vent in the bathroom.
The required postings did not contain the emergency back up person.
The educator completed her hours but there are not hours on file for her assistant.

The licensor observed children in unlicensed space several times during the visit: children were in the unlicensed basement space, children were in the front room upstairs which is not licensed and one child went into unlicensed space to get paper towels for the educator. The children went into the unlicensed section of the basement where there was a double hot plate in use.

The educator was not washing the children’s hands with soap and water after diapering, she is using hand sanitizer.
The basement exit is not accessible or safe. The licensor observed that the room leading to the exit is under construction and the exit is not usuable. There was no path to the exit that was clear or hazard free and the following was observed: boxes, several shelving units, a rug, a door frame, exposed insulation, several large black trash bags filled with heavy materials, baseboard heating, several cords, a nail gun, a bottle of whiskey, an air compressor, a table saw and multiple tools all accessible to children

The Educator’s PQ# expired in 2/2024. The assistants PQ# is also expired
A review of 7 children’s records show that: two children was missing an enrollment packet, 1 child was missing authorized pick up and the general permission was blank, 3 children need updated medicals, 2 children need proof of a lead test.
There has not been a documented fire drill since May 2024..
The first aid kit was missing: tape, gauze roll
There is a new infant in the home who has not been added to the household composition
The Educator did not have training hours for herself or her assistant
The educator is licensed for 6 children. There were 5 children, including the (REDACTED) present at the time of the visit. There are 2 additional enrolled a (REDACTED). The (REDACTED) was not present and the (REDACTED) was present in the morning and scheduled to come back after school. There was an additional child enrolled, whose (REDACTED). From Oct. 11-29 there were 8 children daily in the program, now there are 7 children daily.
The licensor observed that all the children’s belongings were stored together and not individually as required.
A (REDACTED) child went into the kitchen, unlicensed space, unsupervised.
The licensor observed the following: in the bathroom: the trash can did not have a cover. The brick around the fireplace upstairs was exposed
The Educator had no working smoke detectors in the home additionally the smoke detector on the 3rd floor was taken down. The outlets in the bathroom was uncovered and accessible to the children. There is currently not a working stove in the home so a double hot plate was being used in unlicensed space, however the hot plate was left on and unattended and the children went into that space. Outside there were the following hazards: several ladders, a washing machine a snow blower and a pull out couch with no cushions.
The licensor observed a woman on the premises who is there once a month and does not have a BRC
The educator has children in care who require life saving medication and the list is not posted
