In Preschool #4 at approximately 12:00 pm the following was observed: - A small group of children was coloring while eating and playing with each other's hair. -Three children were chasing each other around the classroom running over the children's mats and blankets. -Two children were having a verbal disagreement. -One child was sitting at the lunch table alone crying. -There was a group of children lined up to wash their hands pushing each other in the line. One educator was at the sink and the other educator was putting out mats. Neither educator interacted with the children to provide a safe transition between activities.

The following was observed an accessible to children on the playground: PRESCHOOL PLAYGROUND: -The ground surface was approximately 2 inches thick under the large climbing structure. The use zones under and around climbing structures must be covered wit an adequate depth of an impact absorbing material. The licensee was informed they could not use the large structure until the required amount of ground cover was installed. -There was a piece of the metal border around the equipment that was broken and sharp. -There were large sections of the chain link fence that were not secured to the poles. TODDLER PLAYGROUND: -There were broken plastic toys with sharp pieces. -There was trash on the ground near the fencing. -The large plastic house was broken into pieces. -The large plastic slide was broken. -There were 2 cement bases that hold up the poles for the fencing sticking out of the ground approximately 4 to 6 inches in length. -There was rotten wood surrounding the A/C unit. -There were unknown vegetation growing into the playground near the A/C unit.
