During this visit, licensor observed the following: Upon arriving at the program, licensor observed two infants, one (REDACTED), napping in bounce chairs. Licensor requested that the children be moved to playpens immediately. When the assistant placed the (REDACTED) in the playpen, the child was positioned on their stomach. Licensor instructed the assistant to turn the infant onto their back and reminded them that infants must always be placed on their backs when sleeping, in accordance with safe sleep practices. The second child, (REDACTED) old, was placed in another playpen. During the inspection, the licensor observed that this playpen contained three mattresses and instructed the assistant to remove the additional mattresses. It was also noted that in both playpens, the mattresses were not securely attached to the bottom, and the sheets were loose.
