Reviewed children’s records. One file did not have lead screening indicated. Another file had missing parent signature, signature date, and the developmental history was not fully completed. Reviewed with educator that general permission should indicate children are taken for neighborhood walk.

The diapering table was observed to be easily moveable and needs to be secured. It was observed that the educator has installed a swing set in her backyard. This swing set is not approved for use with daycare children, it does not meet the requirements for fall zones and ground surfacing, of which these were reviewed with the educator. Educator must prevent access to swing set if she wants to utilize her backyard. There was also a ladder and some random items stacked up on the side of the yard. Educator requested to be able to utilize her front yard. It was observed that there was access to shovels, a trampoline, large rocks, and heavy machinery, all are accessible hazards on the front yard.
