It was observed that a (REDACTED) was sleeping in a pack and play with a bib on, there was an extra padded mattress in the pack and play, and there was a boppy pillow attached to the top of one of the sides of the pack and play. Infant woke up during observation and educator removed bib, boppy pillow, and additional mattress. There was a second pack and play observed, educator was asked if that was the pack and play where the (REDACTED) naps, which she stated it was. Although, (REDACTED) was not sleeping in the pack and play at the time it was observed that the sheet on the mattress was not a tightly fitted sheet. Upon review of educator's licensing history and review with FCC supervisor it was determined this is the educator's second safe sleep violation.
