A review of four medications stored at the program indicated the following: -One over the counter medication prescribed for a child's (REDACTED).

Upon a review of 11 medications stored at the program, the following was identified: -One child's (REDACTED) was not contained in its original box with the affixed prescription label as is required. During the visit, the Director stated the program has reached out to the parent regarding this and requested updated medication for the child.
Upon a review of medications stored at the program, the following was identified: -One child that was identified in their IHCP as having been prescribed both an (REDACTED) did not have (REDACTED) stored onsite in the event of a potential medical emergency.

The Building Inspection Certificate expired on 9/1/2022. The Fire Inspection was conducted on 2/3/2021.
Upon a review of Individual Health Care Plans for children enrolled at the program, one child did not have a complete IHCP.
Upon a review of medication stored at the program, one child's prescribed (REDACTED) expired on 3/8/2022.

IHCP (Individual Health Care Plan)
Out of four Children's Records reviewed: -Three records did not have a parent/guardian signature and date on the child's IHCP (Individual Health Care Plan). -One record did not have a complete IHCP, as the description of the health condition, symptoms and side effects of medication, and the person who trained the educator in the administration of medication was not filled out.
Toxic Substances
In the Preschool II classroom, (REDACTED) for two different children had both expired. Therefore, two children in the class did not have access to medication in the event of an emergency.
There was not a staff schedule posted at the Program. The Program must keep the schedule current and posted in an area easily visible to educators, parents and visitors.

A review of three children’s records at the Program indicated: -Two files were missing evidence of Lead testing results on the physical examination form.

The following was observed and accessible to children: -In the Pre-K classroom, a radio on a blue shelf in the circle area had a wire longer than six inches.
