A walk through of the outdoor playground indicated there are several large areas of the rubberized safety surfacing that is ripped/torn which poses as a potential tripping and overall safety issue. The program must ensure that the use/fall zone surface under and around the pieces of equipment (swings, structures, slides, etc) onto which a child falling from or exiting from the equipment would be expected to land is properly maintained to prevent potential child injuries. The program utilizes a public school playground. Technical Assistance was previously provided to the program about this issue on 11/19/24 during Visit#154568. At that time, the program reportedly submitted a work order to the public school facility department for repair.
