At the time of the visit the program did not have the staff files available for EEC to review, the staff record checklist was not up to date, and several of the files did not have the required docummentation.
At the time of the visit the program was out of ratio 2:38 not following the 1:13 or 2:26 requirement.
At the time of the visit the schedule was posted however it was not current.
At the time of the visit the educator was not supervising the children while they were using the bathroom in the hallway.
At the time of the visit the program did not wash their hands prior to serving the pizza to the children.
At the time of the visit the program had (1) expired IHCP and (2) did not have parent signatures.

At the time of the visit the Building and Fire Inspections are expired.
At the time of the visit several of the staff files were missing the following information: Interview, (2) references, orientation, and physical with (2) MMR.
At the time of the visit (1) IHCP was expired, (1) IHCP need a parent signature, the program had an EPI with no name or box and unsure of who the EPI belongs to.
At the time of the visit the program had several educator bags/purses accessible and within reach of the children.

Several children were not supervised when they walked down the hall to the bathroom.
The older group of children did not wash their hands prior to eating snack.
At the time of the visit (1) IHCP was expired and (1) bottle of benadryl is needed for a child with an IHCP.
