The licensor observed the kitchen and it was not clean. Observed grime on the stove, dust on top of the refrigerator, unknown residue on the lower cabinet doors. Also observed clutter located closer to the door, including chemical products, boxes, tools, etc. The wall has some damage that needs repair. The educator requested the kitchen to be removed from the licensed space. Playroom: Three toy shelves not secured to the wall. Please send pictures with response. The licensor observed a small slide on grass in the back yard. The educator stated that daycare children do not use the slide. Please remove it or make it inaccessible. Please send a picture with response.

Pack and play and mats. The educator only has one pack and play and it is shared by two infants. Discussed purchasing another pack and play and always having individual napping materials for each child.

The educator stated that when the infant falls asleep in the swing, she transfers the infant to the pack and play immediately. Licensor observed a thick mat on top of the pack and play mattress. Discussed not modifying sleeping equipment. Issued safe sleep parent letter. Please write corrective action, have parent sign and attached the signed letter to your response.
