The program did not have a method of knowing exactly who is present on the premises at any given point in the day. There were no attendance records with staff that took children to the outdoor space or different groups and, neither was the staff aware of their ratios. The program was keeping one main attendance for all groups. Please ensure the program has a method of knowing exactly who is present on the premises at any given point in the day including while children are in the outdoor space.
The program did not have an up to date building inspection. Please ensure to obtain a building inspection as soon as possible.
All emergency medications were being stored in one of the classrooms. Please ensure emergency medications are immediately available for use and accompany the children anytime they leave the facility in the care of staff.

Documentation of January 2022, February 2022, May through August 2022 were not available for review at the time of the visit. Staff meetings held September 2022 and March 2022 did not include the time each meeting was conducted.
A Background Record Check review reflected three staff with a pending BRC status and one staff with an expired Fingerprint.

Documentation of each child's arrival time was not recorded on the daily attendance for all children attending the program. The program is currently using a check mark system to indicate each child in attendance. The daily attendance was not brought with staff when children transition to other areas of the facility. Four staff from various classrooms were unaware of how many children were present. A fifth staff reported having eleven children, however twelve children were present in the classroom.
Two children's Individual Health Care Plans did not include a signature from the parent.
