Educator's current CPR/1st Aid certificate expired 3/1/25. Educator reported she already took a recertification course on 2/28/25. However, she has not received the new certificate yet. Because of this, unfortunately Licensor had no way to verify that Educator completed the recertification course.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed was in the kitchen. The child safety tab was not working on the small drawer next to the fridge and it had a lighter and small, chokable items inside.

Licensor randomly reviewed 3 of the children's records. Of these, 2 were complete and up to date, but the third had out of date signatures in the enrollment packet.
Licensor randomly reviewed 3 of the children's progress reports. Of these, 2 were complete and up to date, but the 3rd record did not have a progress report.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed was in the bathroom. There were hygiene products in the bath tub that were low and accessible.
Licensor reviewed 4 of the children's records. Of these, 2 were complete and up to date. The other 2 were missing lead testing.

Space (indoor / outdoor hazards) - R Apr 2025
Licensor inspected all of the indoor licensed space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed inside was that there were cleaners (Comet) at the back of the sink in the kitchen. Educator utilizes a fenced in area in the back yard as the children's outdoor playspace. No hazards were observed outside.
Licensor reviewed one of the children's records. All signatures were up to date. Educator had an up to date immunization record for the child, but an out of date physical.

Licensor inspected all of the licensed space and observed the following hazards: **. Kitchen - knife block is on the counter, bleach/water solution also on the counter **. Diningroom - no hazards **. Livingroom - no hazards **. Bathroom - child safety lock on the vanity cabinet is not working and there are hygiene products inside **. Outside - sand box was not covered. The play space has pea stone on the ground. Discussed that children under 2 cannot have access to pea stone as it is a choking hazard. Educator stated [REDACTED] brings the octagonal play yard outside and the younger children stay in there.
This follow-up visit inspection recorded no violations or advisories.
