When licensor walked in the 6 month infant in care was awake. The infant was eventually placed in pack and play and slept for less than 10 minutes. It was observed that the fitted sheet was not the correct size for that pack and play and therefore, was too loose. Additionally, it was observed that there was a thin blanket placed over the mattress as well. Both of which are violation of the safe sleep regulations. The safe regulations and policy were reviewed with both educators. Safe sleep parent letter will be emailed to the educator, safe sleep pledge will be emailed to the educator for both educators to sign, and safe sleep training will be made available for both educators to retake. A follow up visit will be conducted. Educator did immediately change the fitted sheet to the correct one while the licensor was present and removed the blanket as well.
