At the time of the visit, the following was observed: Within the Green Classroom (I/T) there was a play kitchen that not not secured and was tippable. Within the Purple Classroom (PS) there was a toy puppet theater and a doll house that was unsecure and tippable. The trash barrels within the Orange Classroom (PS) and Purple Classroom (PS) were broken, exposing the trash. It was observed that the trash barrels had paper towels within them. Garbage must be kept in lined and covered containers and all trash containers must be emptied at least daily. The Preschool Playground had a rock wall climber designed for students 4-8yrs old. Students within the preschool program are 2.9-5yrs old, the rock wall climber was not age appropriate. There was a slide on the preschool playground and two slides on the Toddler Playground that did not have a use zone of six feet in all directions. At the time of the visit, the Licensee moved all three slides to ensure there was a proper use zone. The use zone should extend a minimum of 6 feet in all directions from the perimeter of the stand alone climber. The woodchips on the Preschool playground were one inch in depth. The woodchips on the Toddler Playground were two inches in depth. The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed and non-tippable.

The following was observed to be accessible to children: -In PRESCHOOL/TODDLER, a large container of disinfectant wipes was sitting on a narrow window ledge, three loose iPod and iPad cords stretching from the outlet to the devices on a window sill above were not secure, the play refrigerator was not secure and tippable, the bookshelf against the red wall was not secure and tippable, and a box of gloves was on the edge of the changing table and considered a choking hazard. -In INFANTS, a music speaker cord was loose and not secure, and the unlocked changing table contained loose bags and bags of wipes which are a choking hazard. -In TODDLERS, the changing table was unlocked and contained loose trash and storage bags, the shelves to the left of the changing table held ointments and bags of wipes, and the wooden bookshelf across from the sink was not secure and tippable. -In the EXTENDED DAY space, outside of the locker rooms, the TV was not secure to the cart and considered tippable, and a young child was using adult scissors.
