The following were observed: -PS room had a piece of furniture with a pink roof that was tippable. -The inside of the paper towel box was dusty/ dirty.
A potty seat was laying on the floor of the bathroom.

During the visit, the I/T room has a preschool child in time out. This put the I/T room at 10 children. When asked, the I/T staff stated they had 10 children. The I/T room did not have all children signed in at the time of arrival. In the PS room, staff responded that they had 12 children, however there were 13 children marked as present.
-There was chipped paint in the PS room near the sink. -there was chipped plaster at the gate between the I/T and PS rooms.
Diapering and Toileting for children under 2.9
A hole in the plaster wall was accessible to children in the Infant/Toddler Room.
Eleven children were listed as present in the Preschool Room; however, twelve children were present. This was corrected at thee time of the visit.
The diapering pad was not secured to the counter. In addition, the covering was creased and folded, preventing sanitizing and disinfecting.

The following was accessible to children; a teachers purse, a can of aeroson spray in the unlocked staff bathroom, a container of Aquaphor, labeled keep out of reach of children, on table in Preschool Room. All hazards were removed at Licensor's request.

It was observed that in the Preschool bathroom, a toilet plunger and push mop were located next to the sink, accessible to children.

It was observed that the hallway leading to the outdoor play area was cluttered with an open pack and play, blankets, cleaning equipment and a chair. Exits and evacuation routes must be kept clear of obstructions.
Emergency numbers need to be posted in an area easily visible to parents, educators and visitors.
The following were observed and accessible to children: BATHROOM - A box of gloves were in the unlocked cabinet under the sink. OUTSIDE - Just outside the door was a broom and dustpan against the wall, a wooden plank and some trash on the ground.
