the following were observed indoors: -The diaper changing pad in one room had small tears, making those areas unable to be cleaned by cleaning, sanitizing and disinfecting procedures for the diaper changing pad. -There was duct tape over a cabinet in one room. It was peeling, leaving the sticky edges unable to be cleaned. In addition, there was sticky residue in the PS room on one shelf.
Outdoors, several climbing structures did not have impact absorbing material (ground surfacing) in their use zone. In addition, the material had large pieces of wood mixed within the surfacing (6 inches plus). There were two pieces of equipment that did not have a 6 foot use zone. (one was 4'1" and the other was 3'4") -One climbing structure was broken, exposing sharp edges.

At the time of the visit, one classroom was over ratio and was only staffed with one teacher. Her co-teacher had taken a 15 minute break and had not asked administration to relieve her.
There was chipping paint throughout the building.
The mulch around climbing structures was not sufficient to protect children in the event of a fall.
In the Pre K room a bottle of hairspray and a bottle marked "disinfecting" solution, marked keep out of reach of children, was accessible to children. In addition, a cord over 6 inches was accessible to children.

The changing surface in one room was torn. In addition several vinyl nap mats were torn.
Staff meeting documentation did not always include the length of the meeting.

The following were observed outdoors: -There were many acorns throughout the toddler playground which could be a choking hazard. -More than one picnic table had eroded wood seats that had sharp and splintered edges. -There was unidentified foliage and vines coming through the fence. -The metal base of posts were rusty. -The latice fence was loose and leaning forward. -Several pieces of equipment were soiled. -

Fall zones adequate depth of impact absorbing material
One slide did not have an adequate fall zone. In addition the mulch had large pieces of sticks and wood throughout. The mulch was not a sufficient depth around climbing equipment.
In the Infant Toddler room educators stated that they did not use a barrier or a covering on the changing surface when diapering children.
There was peeling paint throughout the program.
There was a broom leaning against the counter that was tippable. Children were walking near the space where the broom was leaning. This was a potential trip hazard. In another room plastic bags were accessible to children. The protective plastic piece on one pipe under the sink, was not attached.

A review of 3 children’s files indicated that one did not have current annual paperwork (face sheet, first aid consent, transportation agreement and developmental history).
The following were observed: -I/T: One door handle was broken. -The kitchen was accessible. Inside was a stove and sharp knife in the dish drainer. -The push bar to the back door was broken.
The use zone around the climbing structure did not have an adequate amount of ground surfacing. There were bolts protruding into the playground from the gate/ fence. There was a plastic bag of trash hanging from the fence, accessible to children.
