During the visit it was observed that an educator was cleaning a table then failed to wash their hands after that task.
During the visit, the pea stone did not measure to 9 inches therefore the depth of impact absorbing material was not met.
There was medication missing for a child with an allergy, the plan called for an EpiPen and Cetirizine, the Cetirizine was not in the first aid bag with the EpiPen.

During the visit an educators purse was on a shelf by the door in the butterfly room and it was accessible to children. There was a loose extension cord under an easel in the rockstar room.
During the visit medication was reviewed and in one room there was a child who requires an inhaler with an aero chamber, the inhaler did not have the original box or prescription label.
