Staff members (DOH: 09/27/24, 08/01/25, 12/02/24, 11/01/24, 08/15/24, and 11/04/24) did not complete the required six (6) hours of cabinet-approved orientation within the first three (3) months of employment in a child care program. 2. A staff member (DOH: 08/01/23) completed two (2) of the required fifteen (15) hours of cabinet-approved early care and education training completed between 07/01/24 through 06/30/25. 3. A staff member (DOH: 07/31/23 ) completed eight and one-half (8 1/2) of the required fifteen (15) hours of cabinet-approved early care and education training completed between 07/31/24 through 06/30/25. 4. A staff member (DOH: 04/03/24) did not complete the required fifteen (15) hours of cabinet-approved early care and education training completed between 04/03/24 through 04/02/25. 5.
