General: Based on observation and interview, the health, safety, and comfort of children were jeopardized by the following occurrences: After Care Classroom 1. Upon the DRCC representatives' arrival, the two (2) staff members who were supervising children in the After Care classroom did not express knowledge of the number of children in their care. There were eleven (11) children present; however, when asked how many children were in their care one, (1) of the staff members said there were ten (10) children. The other staff member did not respond. When the DRCC representative stated that she counted eleven (11) children, the staff members then counted the children and acknowledged that eleven (11) children were present. Infant/Toddler Classroom: 1. An eight (8) week old infant was sitting in a car seat. The infant was not secured in the seat with the safety restraint straps. 2. A fourteen (14) month old toddler was asleep on a mat with his/her head and face fully covered with a blanket.

General: Based on observation, children from room 106 was observed sitting on the floor in the hallway while the staff member changed the diapers/pull-ups of children who wore them. The staff member did not wash the hands of the children who had their diaper/pull-up changed after the change was complete. Upon completion of all changes, the staff member used disposable wipes to 'wash' the children's hands in preparation for snack. The children were then served snack. Therefore, children's hands were not washed with running water and soap after diaper/pull-up changes and prior to eating.

Two (2) booster seats no longer had the protective covering for the plastic seat. At least one (1) booster seat was heavily soiled with a discoloration on the surface of the material. One (1) booster seat had the following statement embossed on the underside of the seat: "Do not use this seat after December 2014". The final usage date could not be seen on the remaining seats; however, they appeared to be similar in the amount of wear and tear as the seat with the documented expiration date. Interview confirmed that the aforementioned booster seats have been used by enrolled children.

The surveyor toured the outdoor playground with the staff person in charge and found two wooden play structures that did not have a sufficient amount of fall zone surfacing material underneath. One structure contained a swing and a slide. The second structure contained a slide and a climbing wall for steps. Mulch was visible at the base of each slide; however, the surface around the rest of both structures and under the swing consisted of mainly grass and dirt. The staff person in charge acknowledged the finding and stated that she is considering moving the playground or tearing down the two large wooden climbers; therefore, she has not yet purchased mulch to replenish the area. General: Based on observation, it was found that upon arrival at the facility at 7:50 a.m. there was one (1) staff person with seventeen (17) children in the School-age Classroom. The children ranged in age from five (5) months to three (3) years. The staff person in the classroom indicated that there was another staff person down the hall changing a child’s diaper. Approximately one (1) minute later, the staff person that had been down the hall came into the School-age classroom with another child. Between 7:50 a.m. and 7:54 a.m., two (2) more children arrived at the facility. At 7:55 a.m., one (1) of the staff person took five (5) infants and toddlers to the Infant classroom. This left one (1) staff person with fourteen (14) children in the School-age classroom. The children then ranged in age from two (2) years to five (5) years. Another staff person arrived in the School-age classroom at 7:56 a.m. That staff person stated that she had been in the back of the facility, checking classrooms.
