General: Based on review of documentation, and KY-TRIS, the surveyor discovered the following: 1. One (1) staff member (DOH: 07/01/24) had completed three hours (3) hours of the fifteen (15) required hours of cabinet-approved early care and education training for the review period 07/01/24 - 06/30/25. 2. One (1) staff member (DOH: 11/04/24) last completed the one and one half (1 ½) hours of pediatric abusive head trauma training on 09/25/18. A review of the staff member’s file and a review of KY-TRIS found no additional PAHT training; therefore, the PAHT training is not being completed every five (5) years. 3. One (1) staff member (DOH: 08/20/25) had completed nine and three-quarters hours (9.75) hours of the fifteen (15) required hours of cabinet-approved early care and education training for the review period 07/01/24 - 06/30/25.
