General: Based on review of documentation and ECE-TRIS, the surveyor found that one (1) staff (DOH: 07/29/24) completed the one and one-half (1 1/2) hours of Pediatric Abusive Head Trauma (PAHT) training on 06/05/18; therefore, the training was not obtained every five (5) years as required. During interview, staff-in-charge stated that they had difficulty accessing ECE-TRIS since the shut-down earlier this year and she thought the staff member had completed this training after her employment with the childcare center.
