General: Based on review of documentation, the surveyor discovered a written discipline form in a staff member’s (DOH: 09/04/25) file. The documentation showed that the staff member had violated the child care center’s smoking policy multiple times. The form also detailed an incident in which the staff member’s vape device fell out of her pocket while she was in the classroom, and a child had picked up the device. During an interview with the staff-in-charge, the surveyor was informed that the child who picked up the vape was sixteen (16) months old and had placed the side of the vape device to his mouth. The staff-in-charge stated that due to concerns about possible leakage of the contents inside the vape device, poison control was contacted for further guidance. Therefore, the surveyor was able to determine that the health, safety, and comfort of the child was compromised.

General: Based on review of documentation, the surveyor discovered in a staff member’s file (DOH: 09/15/25) a written discipline form dated 11/05/25. The discipline notice described that the staff member had violated the child care center's smoking policy multiple times and detailed an incident in which the staff member’s vape device fell out of her pocket and a sixteen (16) month old child picked up the device. A child's (DOE: 10/06/25) immunization certificate was no longer current as of 10/15/25. 3. A child’s (DOE: 09/22/25) file did not contain an immunization certificate. An Immunization Summary Report was discovered in the child’s file; however, the document only listed immunizations and did not indicate that the child was up to date on the immunizations. Therefore, the child care center failed to maintain a current immunization certificate for the children.
