General: Based on review of documentation, video footage with audio on 06/30/25 showed a one-year-old child slip on the lid to a sort stacker and fall forward hitting his head on the edge of a low table. There are 2 staff in the classroom, one seated in a chair near the child who fell and the other staff was standing across the room from the child. Both staff were looking at their cell phones when the incident occurred. The child was visibly upset, crying, holding his head and even stomping his feet. The staff person in the chair looked up from her phone and said, "You are fine (child's name)" and the other staff person walked over and said, "You're fine, put your paci in your mouth" then walked away without seeing the injury. Almost a minute goes by with the child still screaming when the staff person who was in the chair got up, walked past the child then turned around to say something to the child and noticed he had a head injury. The child is then picked up and brought over to the changing table for first aid. The child ends up going to the emergency room where glue is placed on the cut. It is important to note that at the beginning of the video footage there is a different one-year-old child who is standing right next to the staff person in the chair and he is crying and touching the staff person with his hand trying to get her attention. The staff person is on her cell phone ignoring the child and then looks over at the child and says in a very agitated, loud voice, "Why are you crying?" "What is wrong with you?"
