General: Based on interview and review of documentation, on 04/15/26 at approximately 3:30 p.m., a one-year-old child exited the playground to the front of the center and was found by a staff member who was pulling out of the parking lot in their vehicle. The child was unharmed and returned to the classroom staff approximately twenty (20) to thirty (30) seconds after exited the playground. An incident report was written later in the day, though the incident was not reported to the parent until approximately 6:00 p.m. after the staff-in-charge had fully investigated the incident, including speaking with staff and reviewing video footage. General: Based on interview and review of documentation, it was found that per the facility's attendance records on 07/14/25, the Older Opening Classroom went over ratio at 7:39 am. There was one (1) teacher present, the youngest child was two (2) years old, and the eleventh child arrived at 7:39 am. Six (6) more children arrived before a second teacher entered the classroom at 7:58 am. The person in charge viewed the records. She stated she was working in a classroom at 7:39 am on 07/14/25, and was unaware the classroom had gone over ratio.

General: Based on interviews and review of documentation, it was determined that a staff member (DOH: 05/29/25) used profane language in the Two-year-old Classroom, in the presence of children, on 05/29/25. Another staff member overheard the language and confronted her, yelling until the staff member exited the classroom. Witnessing staff members reported that children appeared frightened during the incident.

General: Based on observation, interview and review of documentation, the following was found: 1) Between 7:00 AM and 9:00 AM, for several weeks, the center was frequently out of ratio and mixing age ranges. Staff members report that this is due to staff members calling in, arriving late, or leaving just one (1) staff member in the classroom with ratio exceeded while they stepped away or moved to another classroom. Staff report that this occurs most often in the Infant Classroom, One-year-old Classroom, and occasionally in the Preschool Classroom. Staff indicate that they would be over ratio between fifteen (15) minutes up to one (1) hour. 2) The Infant Classroom exceeded staff-to-child ratios on 06/23/25. The classroom exceeded ratio at 7:21 AM when a child arrived. The youngest child in the group was an infant. A seventh child arrived at 8:17 AM and a second staff member joined the classroom at 8:18 AM, bringing the classroom back to ratio. The second staff member left the classroom at 8:25 AM. A third staff member joined the classroom from 8:25 AM until 8:50 AM. Four (4) children were moved to the One-year-old Classroom at 8:00 AM. One (1) staff member cared for six (6) children aged zero (0) to one (1) between 8:55 AM until 9:05 AM.
