The certified family child-care home provider failed to report to the cabinet within twenty-four (24) hours of a notice of a temporary closure for the following dates of January 26th,27th and 28th of 2015. General: Based on Observation and interview with the provider, the family care home failed to maintain equipment in accordance with regulatory requirements. Based on observation at 1:30 p.m., it was revealed that there was one (1) child who was asleep on the floor with no individual bed that was either a two (2) inch thick waterproof mat or cot that was in good repair. Interview with the provider revealed she did have the mats available; however, she did not have clean sheets available for the mats during naptime.
