General: Based on interview and review of documentation, this regulation was not met. On 09/21/23, as class #2 was returning from the playground, a three-year-old child was left outside alone for approximately two (2) minutes. Another adult who was at the center to pick up a child, heard the child screaming and crying. The adult followed the cries up the side ramp and found the child alone. She then entered the side door where she found two teachers with children in the hallway. The teachers were unaware that the child was left outside. General: Based on interview and review of documentation, it was found that on 3/21/19 a four year old child was removed from the classroom and allowed to be taken in the hall for a visit with her paternal grandmother. The child's father's girlfriend was also present during the visit in the hallway. According to staff the grandmother and the girlfriend visited with the child in the hallway for ten (10) to fifteen (15) minutes. A review of the child's file found that there was an Order of Protection regarding domestic violence against the child's father. The child was listed as a protected minor. Further, neither the grandmother nor the father's girlfriend were listed as authorized pick-ups or contacts in the child's file. The facility did not notify the child's mother about the visit until she picked the child up that afternoon. The mother disenrolled the child from the facility as a result of the incident.

General: Based on observation, it was determined this regulatory requirement was not met. 1. The classroom closest to the main entrance had several ceiling tiles that were in need of repair. The tiles above the shelf, near the thermometer on the right side of the building, were loose and the metal framework was observed to be loosely hanging. The ceiling tile above the shelf on the left side of the same wall was bowed outward and not fitting properly against the ceiling. 2. In the second classroom, near the door in the back of the room, the ceiling tiles were dusty and cobweb like dust matter could be seen hanging from the ceiling around the area.
