The surveyor arrived at approximately 10:15 am and walked directly to the playground area. The surveyor observed fifteen (15) children with one (1) staff member. The staff member stated that the children were ages four to nine-years-old (4-9). Therefore, the staff member was over-ratio by one (1) child. She stated that she had been over ratio for approximately ten (10) minutes. She texted the staff member in charge to let her know that she was over ratio by one (1) child. At approximately 10:28 am, the staff member in charge entered the playground and said that she would send an extra staff member outside. At 10:51 am, a second staff member entered the playground. The surveyor then noted that the situation was corrected; there were eighteen (18) children present on the playground ages four- to eleven-years-old (4-11) with two (2) staff members. However, at the time of the visit there were three (3) children in the infant room sleeping on the floor on the cloth mats that are not waterproof and are not two (2) inches thick. An investigation was conducted at the facility due to allegations involving school age children being made to write curse words on paper. Administrative staff stated during interview, "One of the kids has been writing curse words on the back of the van seat and on the bathroom wall, but we don't know who it is. Right after the words were found on the bathroom wall, the staff person working in the school age room sat the children down at the tables and she asked them to write the first couple of letters of the bad words on paper. She did this as an attempt to match a child's handwritting to the words written on the wall". Administrative staff further stated, "We didn't know that she did this until a couple parents called and complained. The staff person told us that she thought it was okay to have the children do this because she went to a catholic school and this is how the school handled this type of situation". A phone interview was conducted with the staff person identified as the one working in the classroom. The staff person denied instructing the children to write the specific curse words on paper; however, stated, "The older children in the room knew why I was having them all sit down and write their name on a piece of paper.
