General: Based on observation, the following was found: 1) The Preschool Classroom had three (3) stacks of cushioned adult chairs stacked four (4) to a stack posing a hazard to the children. 2) The Preschool Classroom had two (2) separate electrical outlets, located on the stage floor, without protective covers. 3) The Preschool Classroom had three (3) long tables leaned against the wall between the exit door and Two's Classroom presenting a hazard to the children. 4) The Preschool Classroom had a round table on the stage. The chairs of the table were positioned at the edge of the stage with children sitting in them. The surveyor had to stop a child from falling backwards off the stage by moving her chair closer to the table. 5) In the Two's Classroom, there were three (3) stacks of three (3), (4) and (5) chairs position against the wall posing a hazard to the children. 6) In the Two's Classroom there was an electrical outlet on the wall adjacent to the door, without a protective cover.

The staff was asked why there were only two (2) books and she indicated that they had more in a back room but she would need to leave to get them. When asked about why they had limited toys, she stated she did not know as she was new there.
