General: Based on observation, the surveyor found that the outdoor play area was currently under construction and that the playground did not contain any play equipment; therefore, the playground was not kept in good repair. During interview with staff-in-charge, she stated that the playground was currently under construction due to new equipment and resurfacing for a fall zone. She also stated that during this time, the head start children did not have access to an outdoor play area and that the child-care center was utilizing classrooms for indoor gross motor space. General: Based on review of documentation of a Child Abuse/Neglect Background Check, the surveyor found that a staff (DOH: 08/01/20) lived in the state of Utah in the last five (5) years. The surveyor was not provided with an out of state Child Abuse/Neglect Background Check or Criminal Records Background Check for the staff person. During interview, staff- in-charge stated that the child-care center did not submit a Child Abuse/Neglect Background Check or Criminal Records Background Check to the state of Utah because the state is a "closed" state. Staff-in-charge stated that the staff person has not worked alone with children and the classrooms have at least two (2) or three (3) staff members in them.
