General: Based on review of documentation, the surveyor found a staff member’s (DOH: 10/14/15) file did not contain documentation of a current TB skin test or a statement from a health professional stating that the adult was free from active tuberculosis. The surveyor reviewed a TB test that was dated 03/31/22. Staff-in-charge called the Director while the surveyor was present at the child-care center to see if an updated TB skin test could be found for the staff member. The surveyor was informed that the staff member had an updated TB skin test; however, she would need to retrieve a copy from the doctor's office. Staff-in-charge asked if the surveyor could wait until 1:10 pm for a copy of the staff member’s current TB Skin test. The surveyor stayed at the child-care center until 1:30 pm and the TB Skin test had not been sent to the staff-in-charge while the surveyor was present. After the surveyor left the child-care center, the surveyor received a text message at 1:37 pm on her work cell phone from the staff-in-charge of a picture of the TB Skin test for the staff member. The surveyor observed the picture of the staff member’s TB skin test with a date at the bottom that read 01/24/24.

General: Based on observation during a tour of the childcare center, the surveyor found the following: 1. Upon arrival to the childcare center, the surveyor and the staff-in-charge heard a staff in the preschool room yell in a loud and demanding voice for a child to get out of an activity center. The surveyor stepped into the classroom to find that the staff was yelling at children whose ages ranged from two to five-years-old. The staff-in-charge asked the staff to lower her tone of voice. 2. The surveyor found that a staff person diapered an infant child and washed her hands in a sink that contained a child’s bowl and spoon. The staff person then proceeded to rinse out the bowl and spoon using a brush and running water.
