General: Based on observation, when staff member left the Infant Classroom to answer the door for the surveyor, the classroom was no longer compliant in staff-to-child ratio. This left one (1) teacher and six (6) infants in the classroom for over a minute. When staff realized she was out of ratio, she subsequently finished walking with the surveyor while holding an infant.

General: Based on observation, this regulation was not met. In the Infant Classroom, two (2) staff were present. The surveyor asked Staff Number One her name and how many children were present. Staff Number One said seven (7) children were present. The surveyor only saw four (4) children, all asleep on cots. Staff Number One went to Staff Number Two and then went to the back of the classroom behind a curtain. The surveyor then went to the back of the classroom and found four (4) infants behind the curtain. Two (2) infants were asleep, and one (1) infant was awake in the cribs. Staff Number Two was holding an infant. The surveyor asked Staff Number Two how long she had been in front of the curtain. She said she went to ask the other staff member a question. She could not determine how long she had been in the front of the classroom, leaving the infants behind the curtain unsupervised for an undetermined amount of time. General: Based on interview and review of documentation, the child care center failed to maintain state regulations. During the survey, the following was observed: 1. One (1) staff member (DOH: 08/21/2024) did not have an eligible for hire status from a KARES background check. It should be noted the staff member did have a CAN check that was cleared as of 05/30/2023. The staff in charge indicated they were not aware that a KARES background check was necessary. 2. One (1) staff member (DOH: unknown) was present and working with children in care, though they did not have a personnel file available with a current, eligible for hire status on their background check. The staff in charge stated that the staff member had just turned seventeen (17) and a CAN check was not provided either. It should be noted this staff member was working in various rooms preparing lunches for children in care. The staff in charge indicated that this staff member had once been a child in care at a previous center the staff in charge had owned and therefore had "been there forever."

General: Based on observation and interview, the child care center failed to maintain state regulations. The Infant/Toddler combined classroom contained twelve (12) children ranging in age from three (3) months to twenty-three (23) months. At the time of survey, there were three (3) staff members in the classroom, though one (1) was only present to prepare lunch items. The two (2) staff working in the classroom identified the children present as seven (7) toddlers and five (5) infants. Based on the youngest child's age of three (3) months, the ratio of this classroom should be one (1) staff to five (5) children, for a maximum of ten (10) children causing the room to be over ratio by two (2) children. It should be noted the room was sectioned into an infant side and a toddler side, though at the time of survey the three (3) month old child was lying on a blanket on the floor of the toddler side of the room, thus the room was being utilized as a combined classroom and not two (2) separate spaces. The staff in charge later indicated she was unaware of the children using both sides of the room and believed that one (1) year old children could not be in a classroom with (2) year old children which led to the number of children present in the Infant/Toddler classroom.
