Review of additional photographs posted to the same social networking site revealed in another picture a child was shown with food placed in her mouth by a staff member in a manner in which the child could have choked while the child was sleeping with a comment of "…Lesson one children: Never sleep with your mouth open when you are in the room of me and (staff member). Another picture revealed a child with mulch stuck to his face, with the comment, …"He fell flat on his face while we were outside and when he rose up this is what he had on his face. #haha bad thing was, he sniffed one of those pieces of mulch up his dang nose...and guess who had to go get it out?! #muah". Interviews revealed that staff members admitted to staging the pictures by putting food in the child's mouth, placing shaving cream on the children's faces, and by allowing the mulch to remain up a child's nose long enough to take a photograph. Staff confirmed that they took the pictures and were responsible for posting them on the non-secured social networking website for public view. Based on interview and review of the photographs, it could not specifically be determined when the photographs were posted to the Internet, however it was confirmed that the primary responsible staff person departed employment on 5/9/13.

General: Based on Observation, Observation of the large playroom revealed nine (9) two-(2)-year-old children playing on the large indoor jungle gym. Further observation revealed none of the children were wearing shoes or socks. During interview staff stated that the children always took their own shoes and socks off during the day so they did not worry about putting them back on the children. Observation of the Three-year-old room revealed three (3) children were not wearing shoes or socks. Further observation revealed small toys, blocks and Legos on the floor where the children were walking and playing. Observation of the Toddler room revealed four (4) children that were not wearing shoes or socks. Further observation revealed small toys and blocks on the floor where the children were walking and playing. During interview the director stated she was aware that children in the facility did not wear shoes and socks and was aware that staff in the building had not required the children to wear shoes or socks. The director stated she was unaware that it was a health, safety and comfort issue for the children to play without wearing shoes or socks. Observation of the Infant room from 8:10 a.m. to 8:45 a.m. revealed an infant slept in an infant swing. During interview the director stated she was unaware that staff in the Infant room allowed a child to sleep in an infant swing.
