Observation of the playground revealed one of the side panels of the reddish wooden play house was partially detached from the rest of the house. Continued observation revealed at least three (3) wooden boards from the house, each approximately a foot long, were detached from the house and scattered inside and outside of the play house. Further observation revealed each of the three (3) wooden boards had two (2) nails protruding from the end which were accessible to children in care.
