General: Interviews and review of documentation revealed that a staff member was caring for five (5) children when a three-year-old child walked into a pole causing a large bruise/bump on the child’s forehead. Interviews revealed the only first aid rendered to the child was that the child was given an ice pack to apply to their own forehead, which they did for about five (5) minutes until they ran off to play. Interviews and review of documentation revealed that the incident occurred around 11:15am and the parent was not notified until around 2:20pm. Interviews revealed that the child was allowed to lay down for naptime after receiving a head injury where a bruise was visible.

General: Interviews and review of documentation revealed that a three-year-old child received a head injury due to walking into a pole around 11:15am and the parent was not notified until around 2:20pm. Interviews revealed that the staff member caring the aforementioned child along with four (4) other children did not have time to immediately contact the parent even though a bruise was visible on the child’s forehead. Interviews revealed that the staff member was preparing for lunch and naptime, which they stated, prevented them from immediately contacting the parent.

General: Based on observation, review of documentation, and interview, the child care center failed to maintain employee records in accordance with regulations. In the Head Start Room, a staff member hired 7/7/17 was the caregiver, along with another staff member, of twelve (12) children ages three-years-old to four-years-old. Review of staff files revealed the staff member hired 7/7/17 listed a South Carolina and North Carolina address on her Child Abuse & Neglect Check (CAN) signed 7/7/17. A South Carolina Criminal Records Check (CRC) and North Carolina CRC were not presented for review at the time of survey for the staff member hired 7/7/17. The staff-in-charge revealed a South Carolina and North Carolina CRC were not available for review at the time of survey for the aforementioned staff member.
