General: Based on observation, the following was found: 1.) During review of camera footage from 12/10/25 at 11:43 a.m., it was found that children in the Pre-Kindergarten Classroom were directed to sit at a table and crumble a brown piece of paper for eleven (11) minutes. The children are not offered any instruction other than to crumble the paper as the staff member scrolls on their phone and shuffles through paper. Many of the children grow restless from this activity and begin to throw the crumpled papers across the table. 2.) During review of video footage, it was found that a three-year-old child was directed to stay in time-out from 11:05 a.m. until 11:15 a.m. on 12/10/25. General: Based on observation, the following was observed from video footage provided from 12/10/25: 1.) At 11:43 a.m., a staff member in the Pre-Kindergarten Classroom directs children to sit at a table for an activity. Several of the children go to retrieve painting smocks. The staff member yells loudly, “DID I SAY GET AN APRON? I SAID GET A CHAIR AND SIT DOWN!” 2.) At 11:52 a.m., children from one (1) table stand up and begin to put on painting smocks. Staff #5 gives each child a piece of white paper, except for one (1) child. The child throws an item into the air, and a staff member lunges toward her and yells close to her face, “STOP THROWING STUFF!” 3.) A three-year-old child stands near the classroom entrance at 11:56 a.m., while a staff member provides paint to each child. The child walks to the practical living center to play while her peer’s paint. The staff member walks to the center, grabs the child by her left arm and attempts to guide her to time-out. The child refuses to walk, the staff member grabs her by both arms and forcefully places her in the time-out chair. 4.) At 11:56 a.m., a staff member can be seen in the hallway yelling at children in the class across from the Pre-Kindergarten Classroom to “SIT DOWN!”. 5.) At 11:58 a.m., a three-year-old child stands up and says, “I’m not in time out!” The child walks toward the entrance of the classroom. A staff member opens the door as the child appears startled and attempts to back away. The staff grabs her by her right arm and she falls to her knees as he drags her into the hallway forcefully. The staff member can be seen pulling the child to the front of the building by her left arm and she attempts to keep up. The child later reported to their guardian that this staff member had scratched her hand during this interaction.
