General: Based on interview, the child care center failed to maintain a child care program that assures each child shall be provided with adequate supervision, at all times, by qualified staff who insures the child is within scope of vision and range of voice. It is unknown how long the child was in the hallway alone, but was estimated by a staff member that the child was in the hallway for one (1) minute.
