General: Based on interview and review of documentation, a staff (DOH: 03/25/26) was listed as “Separated” on her KARES background history and was separated on 12/06/24 by a previous center. A new application was not completed upon the staff being hired at this center; therefore, her eligibility was not detemined prior to employment. The staff was working in the Ones Classroom alone on the date of the inspection. After leaving the center, they submitted a background check application and it was determined that new fingerprints were needed to determine current eligibility.
