A staff’s (DOH: 03/31/22) file contained a copy of a Child Abuse/Neglect Background Check (CAN) that was sent on 03/31/22. The CAN check verified that the staff person lived in Ohio in the last five (5) years. During interview, staff-in-charge stated that the staff person was seventeen (17) years of age. Staff-in-charge stated that the staff person has not worked alone with children. The surveyor did not observe the staff person working alone with children. Staff-in-charge stated that she had not submitted a background check to the state of Ohio for the staff person. 2. A plunger located beside the toilet in the restroom in the Infant Classroom. The plunger was observed to be accessible to the children. 3. Two (2) staple removers, one (1) pad, and one (1) tube of Triple Antibiotic Ointment located inside a cabinet drawer in the Infant Classroom. The safety lock was observed to not be latching properly. 4. A large white board leaned on the wall in the Dining Room that did not appear to be secured. The surveyor observed a small gray table placed in front and on the end of the white board. 5. A container of Air-Dry Clay placed on a shelf in the Dining Room that stated “Not suitable for children under thirty-six (36) months due to small parts which may cause a choking hazard if swallowed.” 6. Two (2) Germ-X bottles placed on the sink located in the Two Year Old Classroom that stated “Keep Out of the Reach of Children” on the labels. 7. One (1) tube of Eczema Relief Cream, one (1) tube of Aquaphor Healing Ointment, and one (1) tube of Boudreauz Butt Paste located inside an unlocked drawer in the Two-Three Year Old Classroom that stated "Keep Out of the Reach of Children" on the labels. 8.
