General: Based on observation, the surveyor found the following: Restroom #3: 1. An open canister of white paint placed on the floor. 2. Two (2) cans of unopened white paint placed on the floor. 3. A five (5) gallon bucket of primer placed on the floor. 4. Two (2) pans of paint placed on the floor. 5. Several paint rollers and paint brushes placed throughout the restroom. 6. General: Based on interview, the surveyor learned that a seven-year-old child exited the child-care center on 07/22/24. The surveyor measured the distance the child walked outside and found the length to be approximately three hundred twenty-five (325) feet on the child-care center property. Staff-in-charge stated that a staff member escorted the child to the restroom. When the child finished in the restroom, instead of going back to the classroom, the child exited out the side door. Staff stated that a phone call came into the center asking if they had a child missing. Staff realized the seven-year-old child was missing, she was located outside the center and had been outside for up to ten (10) minutes. When the staff member got to the child, the child was not crying or upset. Staff-in-charge reported that she has installed exit door alarms, and all doors are locked.
