General: Based on observation, the surveyor found a child that was washing his hands at a sink that was located between two (2) walls near the Three Year Old Classroom; therefore, the child was not within scope of vision. The child was observed to be alone and without supervision. The Three Year Old Classroom was observed to have a half wall and a staff person was observed inside the classroom. The staff person was observed to be tending to other children and had her back turned. Another staff person was observed to see the child at the sink and called for the staff person inside the Three Year Old Classroom to look at the child. Through interview with staff, the surveyor learned that the child was three- years-old and that he had went to the restroom alone.
