General: Based on observation and review of documentation, the health, safety, and comfort of children were jeopardized based on the following observations: Toddler 1B Classroom: The changing pad on the diaper changing table did not adequately fit on the surface of the table, resulting in the pad resting on the edge of the table on one side and hanging over the edge on the opposite side. Electrical Room (adjacent to Hallway): A note was posted on the door of the electrical room that stated, "Per Fire Marshal's instruction, this area is to remain clear of any obstructions so access to power box is easily accessible in case of emergency". The door of the electrical room was open and the electrical box was observed on the wall in the back of the room. Although items were stored in the room, it appeared as if there was an effort to create a path to the electrical box; however, the pathway was not clear of obstructions as an industrial mop bucket and mop were stored in the pathway, blocking access to the box.

General: Based on observation, the surveyor found that the Toddler 2 Classroom was not well lit during the surveyor's initial walkthrough at 11:55 am while conducting a head count of the children. When the surveyor first walked in, the surveyor was unable to see children in the classroom, had to use a flashlight, and assumed there was not a staff person present until the staff person stood up from behind a book shelf; therefore, the child-care center failed to keep the classroom well lit at all times. When the surveyor returned to the Toddler 2 Classroom at 12:22 pm for the classroom inspection, the staff person had turned on a night light to make it easier for the surveyor to see around the classroom.
