General: Based on review of documentation and interview, the surveyor found a current staff member (DOH: 07/01/2005) was listed on the child-care center’s Kentucky Applicant Registry and Employment System’s (KARES) report under separated employees with an eligible/expired determination date of 7/23/2018. The surveyor observed the staff member working with the children on the day of the surveyor’s visit. The staff member’s file contained an updated KARES report showing the staff member had a determination of eligible for hire on 5/30/23. The staff member’s individual KARES history shows the staff member was auto determined to be eligible for hire on 5/30/23. Therefore, the provider did not maintain the current employment status of the staff member. During interview, staff-in-charge stated she will contact her central office to let them know the child-care center’s KARES report needs to be updated. General: Based on observation, the surveyor found that the protective surface padding on the playground had a large crack that followed down the side of the playground to the end of the fence area; therefore, the playground was not kept maintained and the playground was a tripping hazard. General: Based on observation and interview, the following was found: (1) One (1) infant was placed in the crib with a loose untucked blanket covering them. Staff in charge reported that staff that typically puts the child to sleep did not do so the day of the survey and the other staff must have forgotten. General: Based on interview, the child care center does not currently offer a variety of creative activites including an indoor or outdoor play area in which a child makes use of both small and large muscles. A staff person (DOH: 06/10/2002) stated that the child care center children do not currently have a gross motor area for play. The children at the child care center do not use the outdoor play area due to the awning that was on the playground had been torn down by a storm.
