General: Based on review of the Kentucky Applicant Registry and Employment System (KARES), the surveyor found that a staff (DOH: 10/01/20) did not have a completed background check. Based on review of the applicant history report on KARES, the surveyor found that an application was started on 09/24/20 and withdrawn on 07/14/21 with the reason stating “other.” The report verified that the application was closed on 08/28/21 and that the staff person did not submit to fingerprints. The staff’s file contained a completed Criminal Records Background Check (CRC) dated 09/14/20 and a FBI check dated 09/14/20. The staff’s file did not contain a completed Child Abuse/Neglect Background Check (CAN). The surveyor did not observe the staff person working alone with children. During interview, staff stated that the staff person has not worked alone with children. Staff stated that a background check was completed through the school system for the staff person. Staff-in-charge stated that she would submit an application for the staff person on KARES. General: Based on observation, the surveyor observed the children and staff in Classroom 2 to leave the classroom to go outside at approximately 10:35 a.m. and return at approximately 11:00 a.m. When the surveyor stepped outside within a few minutes of the children and staff leaving the classroom, the surveyor did not observe the children or staff on the playground or within proximity of the child-care center. Through interview with staff, the surveyor learned that the children and staff took a buggy stroll around the school. Staff stated that the children and staff will go around to the front of the school, circle the softball field, and come back to the child-care center. Staff stated that the children and staff stay on school grounds. Based on review of documentation, the surveyor learned this was not licensed space. During interview, staff stated that parents will circle "yes" or "no" on the sign in/sign out sheet during drop off in the mornings regarding if they allow their child to go on the buggy stroll. Based on review of the sign in/sign out sheet, the surveyor found a section that stated, "Please circle yes if your child is allowed to go on a buggy stroll or no if your child is not allowed to go on a buggy stroll" for each day. The documentation did not contain specific times or locations. Staff stated that this was the only documentation pertaining to the buggy stroll.
